Groups is looking for an organized and self-motivated Justice Involved Case Manager, who will be responsible for providing administrative support to our criminal justice stakeholders and referral partners.

This role works closely with the Growth & Development Team and State Leadership to ensure that Groups is providing high quality care to some of our most vulnerable community members.

To be successful in this position you should be detail-oriented, professional, able to solve problems independently, have excellent written and verbal communication skills, have exceptional customer service skills, and demonstrate the ability to navigate and problem solve under pressure.
Empathy, efficiency, and ability to balance multiple priorities are essential.

The Justice Involved Case Manager will work to maximize the communication with all criminal justice stakeholders (CJS) and ensure ease of access to treatment for residents leaving the justice system.

This team member serves as the "familiar face" that the CJS can connect with, and will be their go-to "problem solver.

The Justice Involved Case Manager will also assist the care delivery team with project management as needed.
Responsibilities
Outreach and Development

Serve as a primary contact both virtually and in-person for the Department of Corrections and all criminal justice partner contacts including but not limited to the jail system, parole, and probation

Provide outreach and connection for criminal justice residents while working in collaboration with the Recovery Support Specialists and other cross functional teams
Engage with releasing residents and stakeholders to ensure they are supported throughout the intake and admission process
Partner with the Growth & Development Team surrounding expansion of criminal justice partnership throughout new markets

Criminal Justice Referral Support

Create and maintain a statewide tracker for all criminal justice system referrals to ensure they do not "fall through the cracks" and are supported throughout each stage of their recovery
Partner with the local Regional Directors, Sr. Office Managers, and Office Managers for scheduling purposes regarding members who need in-person assistance, including technological support
Support continuity of care for DOC referralsCommunicate directly with criminal justice referrals/members to assist with the intake process, coordination of care, and re-engagement when appropriate
Document all care management services in electronic health record
Maintain and reporting requirements of the criminal justice stakeholders
Coordinate care with criminal justice stakeholders including DOC, probation, parole, and DCS while maintaining HIPAA regulations
Assist clinical team with ROI management and reporting to criminal justice partners as needed

Project Management

Create long and short term plans, including setting targets for milestones and adhering to deadlines
Delegate tasks on the project to employees best positioned to complete them
Adjust schedules and targets on projects as needed

Qualifications

Bachelor's degree
2-5 years experience working directly with or in the criminal justice system
Case Management experience (preferred)
